Add the silken tofu, cocoa powder, maple syrup, and salt to a high powered blender. Blend on high until smooth and creamy. (Make sure it is fully blended and smooth, if your blender isn't super high-powered you may have to blend it a little longer.)
Melt the chocolate using either a double boiler or by microwaving in 30 second increments, and stirring until melted.
Add the melted chocolate into the blender with the creamy tofu mixture, and blend again until fully combined. If it gets stuck, just stop the blender and scrape down the sides to make it blend again. You may have to use a slower setting on the blender as it will be getting fairly thick.
Taste the mixture to ensure it's sweet enough for your taste. If not, add another tbsp of maple syrup or sugar.
Transfer into ramekins or wine glasses, and refrigerate for at least 30 mins. Sprinkle your favourite toppings on top, and enjoy!

If your tofu seems extremely watery, allow it to sit on a paper towel, or squeeze the liquid out gently with a tea towel before adding it to the blender. Optional toppings:

Fresh berries
Vegan whipped cream
